Problem: As a student nurse, I was assigned to a clinical rotation in a long-term care facility. During my first week, I noticed that many of the residents were experiencing pressure ulcers, which were not healing despite multiple interventions. The facility was short-staffed, and the nurses were overwhelmed with their workload.
Collaborative Solution: I discussed my concerns with my clinical instructor and the nursing staff. We decided to form a wound care team that included the nursing staff, a wound care specialist, a physical therapist, and a dietitian. We met regularly to discuss each resident’s progress and to revise their care plan accordingly. The wound care team also provided education to the nursing staff about prevention and treatment of pressure ulcers.
