BSN-FP4001 Assessment Community Health Care Resources
Providing care to vulnerable populations in the community can be a challenge. As health care providers, we are empowered with the ability to assist these populations in better managing and improving their health and quality of life. Knowing the appropriate and available resources for these populations is key to accomplishing these goals.

The Patient Outreach Department is responsible for identifying the people in need and what those needs are, by doing this we will help them accomplish the goals of health management and improved quality of life.
Identifying Populations at Risk In our community, Seniors (65 years+) make up approximately 18% of the population. Of that number, 5% are considered to be in poverty (Howard County Maryland, 2017).
Howard County, in cooperation with Howard County General Hospital, has established programs that are available to identified persons in need. By identifying and connecting with these people, we can improve the outcomes of a population that has, in the past, been in a never-ending cycle of poor health and decline.
“Approximately 80% of older adults have at least one chronic disease, and 77% have at least two. Four chronic diseases—heart disease, cancer, stroke, and diabetes—cause almost two-thirds of all deaths each year” (National Council on Aging, 2017).
Many of these older adults have multiple admissions to the hospital each year, usually for reasons that would be avoided with proper health management.
They would prefer to receive appropriate services and care in their homes or in their communities, avoiding the possibility of transition to a nursing home.
For many, this would lead to loss of independence, loneliness, or even death (Palmer, 2014). By assisting this population with that process, and also educating them on the importance of health
